Medical Exemption Appointments

As a board certified Pediatrician, Dr. Allen offers in depth vaccine consultations to help parents better understand vaccines and develop a customized vaccine schedule. He respects families beliefs and choices regarding vaccines, and is here to help you formulate the best approach for your family based on your relevant needs.

We offer medical exemptions ONLY to our established patients who qualify due to VALID health reasons determined by Dr. Allen.

Please read below for our existing and new patient policies.

EXISTING PATIENT POLICY FOR MEDICAL EXEMPTION CONSULTS

If you are already an established patient, and would like a Medical Exemption consult, please call and schedule an appointment  with Dr. Allen or one of our Nurse Practitioners. During this appointment our providers will discuss your comprehensive family health and medical history. After all the relevant medical information is gathered, Dr. Allen will review it and determine if the individual qualifies for a medical exemption.

NEW PATIENT POLICY FOR MEDICAL EXEMPTION CONSULTS

  1. If you are not already an existing patient, you MUST schedule a full New Patient Consult Appointment  (1 – 1 1/2 hrs) with Dr. Allen or our Nurse Practitioners to establish care. During this appointment a comprehensive  individual medical history and exam is performed, and our providers will discuss your comprehensive family health and medical history. After all the relevant medical information is gathered, Dr. Allen will review it and determine if the individual qualifies for a medical exemption.
  2. NOTE: All new patient medical exemption appointments require a deposit  to confirm your appointment. This deposit will be applied to your visit, or used to pay our cancellation fee if the appointment is rescheduled or cancelled less than 7 days prior to the appointment.

There are no exceptions to this policy.
